Emergency Medical Care: If we believe your dog/cat needs medical care and we are unable to reach you, please indicate on the electronic reservation form whether you want us to take your dog/cat to a veterinarian or animal hospital.

By checking YES, you agree to be solely responsible for the payment of all medical bills for your dog/cat and you release Furry Friends Academy, Inc, its officers, directors, agents and employees from any and all responsibility for, or claims, damages, debts, arising out of or related to such medical care, including, but not limited to, transportation to/from the veterinarian clinic and our choice of veterinarian or animal hospital.

By checking NO, you agree to release Furry Friends Academy, Inc from any and all responsibility for or claims, damages, debts arising out of or related to Furry Friends Academy, Inc not providing or obtaining medical care for your dog/cat and you acknowledge that Furry Friends Academy, Inc is not required to give any medical aid.

Furry Friends Academy, Inc reserves the right to change your dog's boarding requirement if we believe it is necessary to protect the health and well being of your dogs and/or staff.

Your response to this release and the 5-digit zip code verification indicates your agreement with the terms herein and certifies the accuracy of the information provided by you herein. You are hereby releasing, indemnifying and holding Furry Friends Academy, Inc harmless from any and all manner of injuries, damages, claims, losses, liability, costs or expenses, causes of action or suits, whatsoever in law or equity (including, without limitation, attorney's fees and related costs) arising out of or related to the services provided by Furry Friends Academy, Inc, including but not limited to destruction of property, dog bites and transmission of disease, except which may arise from the sole negligence or intentional and willful misconduct of Furry Friends Academy, Inc.
